Insurance/Rates
I am in network with the following insurance companies:
**If I am not in network with your insurance, you are welcome to use your out of network option to see me. Just ask me how!**

- Kaiser (PAR referred only)
- MODA
- MHN/Healthnet
- Pacific Source
- Providence Behavioral Health
- Regence Blue Cross Blue Shield
- United Health Care for Nike employees only (I am out of network with UHC however, if you are a Nike employee they will reimburse you as though I am in network. Please ask me if you have questions).

Fees
Payment of your copay, deductible, or session fee is due at the time of service. You will need to have a credit/debit card on file for billing purposes; your fee for each session will be charged to your card within 24 hours of our session unless other arrangements have been made in advance. If you have a balance, it must be paid in full before your next appointment or your appointment will be canceled 24 hours prior. Once you have paid your balance, you are welcome to schedule again.
Cash Pay Rates
I charge the following for sessions. I do offer a sliding scale for those in need of financial assistance. Please contact me to discuss further.
Initial Assessment/Diagnostic Evaluation: $185
Individual Session/Follow-Up Sessions: $165
30-minute Follow Up Session: $90
Comprehensive ADHD assessment/evaluation:
This includes all paper and computerized assessments, clinical interviews, phone calls with collaterals (if indicated), interpretation of assessment results and report writing, development of a personalized treatment plan with science-informed treatment options, collaboration (if needed and requested by the client) with doctors, providers, school, employer, etc., and if indicated, ADHD specific resources and referrals.
The out-of-pocket cost for this portion of the assessment is $155, due before the first appointment. You will receive an invoice for this amount and it must be paid in full 24 hours before the first session, otherwise, the session will be canceled.
Without insurance, the cash pay rate for session #1 is $185, due at the time of the evaluation
Without insurance, the cash pay rate for session #2 is $165, due at the time of the appointment.
**Occasionally a client/family may need or request a third appointment to complete this second half of the evaluation. If a client/family would benefit from a third session, this appointment is also covered by insurance (the insured being responsible for the copay, etc.).

Without insurance, the cash pay rate for session #1 is $185, due at the time of the evaluation
- Session #2 Review of Assessment & Evaluation Results and Recommendations
Without insurance, the cash pay rate for session #2 is $165, due at the time of the appointment.
